Insurance Plans
We
are participating providers for a variety of health insurance
programs.
If
your insurance plan is not listed here, please call our office.
Our list is always expanding.
Please
bring your insurance card with you at the time of your visit so we
may verify your participation. Co-payments must be paid at the
time of service. Credit cards will be accepted. If your
medical bills are to be covered under Worker's Compensation or auto
insurance, please inform our office at the time of your visit.
You must bring with you the authorization for your evaluation and
treatment.
